We no longer supply collision reports.

As of 1 October 2026, the way we process requests for release of road traffic collision information is changing, and we will no longer accept payment via BACS or cheques. We may occasionally change and improve the process outlined below. Please continue to check this page prior to submitting future requests.

How to apply for third party details/finalisation of the case

Please print the 'Request for disclosure of third-party details and finalisation' form below, complete, and email it to [email protected]

Costs

Upon our receipt of this form, you will be notified of the relevant fee and the reference to use when making payment via a link. Do not send any payment with the form at this time.

Following receipt of your payment, we will search our record management systems to find any information available that can be released. All requests will be responded to by secure email and password protected.

How to apply for additional information

You can only apply for further additional information, if applicable, after you've received third-party details as well as the finalisation decision from us.

There are extra fees for additional information, which can include the following:

• list of witnesses/passengers not injured (if available and consent has been given to release their details via South Wales Police)
• witness statements (if available and consent has been given to release)
• photographs (if available and redacted)
• any other documentation that may be available and is dependent on our records (if applicable)
