Q1. Have any police officer in you force been disciplined since 1/1/2013 for either failing to complete a DASH and/or falsifying a DASH.

We have consulted with our Professional Standards Department who have provided the following information in relation to your request.
Yes – 4 officers
Case 1: Related to unsatisfactory consideration of evidence. 2 officers were placed on Unsatisfactory Performance Procedures and placed on attachment with the Domestic Abuse Unit to support understanding of domestic abuse and associated risk.
Case 2: 2 officers received Management Action for their performance which included: attachments to Public Protection working alongside Risk Assessors, interviewers and with training on file preparation, monitored by PPU and Line Managers, to improve the content and quality of PPN and DASH forms, plus wider education related to evidence gathering; CPS evidential requirements; statement quality and file quality; role of body-worn video in assisting Criminal Justice System partners in delivering best evidence.
